WHAT IS MEDIFAST?

MEDIFAST is the specialist general medicine service offered by Venetalab s.r.l. It consists of a regional network of general practitioners and nurses available to respond to requests for medical assistance at home or at affiliated clinics. Doctors perform accurate general medical services, provided quickly after booking, which can be made by telephone, on the website or via mobile application. The performances are indicated for a global assessment of the state of health. The Nurses support the Doctor in the organisation, in the practical management of the patient and in the PAI. WHAT DO MEDIFAST DOCTORS DEAL WITH? The service will follow the indications of good medical practices thanks to which the Doctor will be able to carry out: - reading of clinical tests - collection of clinical history - measurement of vital parameters - objective examination, with reference to specific organs and systems (without the use of electro-medical instruments) - physiological and pathological anamnesis - request medical prescriptions on a white prescription to be used for a fee at the National Health Service or by consulting your GP, it can become a red prescription - illness certificate HOW DOES IT WORK? HOW IS THE SERVICE PAID? REV 1

Payment will be made in advance of the visit, online, and the payment receipt will be shown to the Doctor at the time of the visit.

WHERE IS IT LOCATED AND WHAT CATEGORIES DOES IT HAVE?

The service is currently aimed at the resident or tourist population of the most populous Venetian provinces with an expected extension to the entire Veneto Region. MISSION The general medicine service aims to provide healthcare when, for any reason, the user wishes to have an opinion or a medical service (visit) from a healthcare professional who is not his or her doctor or emergency doctor, which however remain the main point of reference for the user's health. It is useful for non-emergencies and when the medical professional does not have to use electro-medical instruments.

THE VENETALAB - MEDIFAST PROJECT

Venetalab – Medifast carries out the global assessment, defines the clinical care condition and identifies the healthcare needs of elderly people, identifying the most suitable responses to satisfy them and favoring, where possible, their maintenance at home through a Project welfare. This Project must take into account the needs, expectations and priorities of the patient and family members, lost and residual abilities, recoverable functions, as well as personal, environmental and family factors. The Project is: for the subject to be evaluated the right to have personalized treatments based on the centrality of the person, for the services providing the duty to exceed equal services for all and the start of interdisciplinary team work in place of divided interventions and disaggregated. Therefore it must contain the objectives and results that are intended to be achieved, the type, method, location and level of intensity of the interventions. Venetalab - Medifast is responsible for the contents of the Project and usually indicates a contact person for the case. The case contact person is a social or healthcare professional figure, who acts as a point of reference for the non-self-sufficient elderly person and his family, from assessment to taking charge, facilitating, monitoring and coordinating the care path, thus ensuring its continuity, in order to guarantee the adequate socio-health response within the network services, consistent with the project defined by Venetalab – Medifast. The contact persons are identified as the general practitioner and the professional nurse. The Individual Care Plan (PAI) is the operational transposition of the Project and comes developed by the team that takes care of the person. It, starting from the project drawn up by Venetalab - Medifast and after observation and acquisition of any other useful information, defines: - the specific actions, the type of services and the professional figures involved, - the frequency and duration of every intervention necessary to achieve the objectives, - the tools identified for the definition and management of the P.A.I., - the periodic evaluation of the results, with indicators and times, - the congruity between needs and P.A.I. and between new needs and changes to the P.A.I. Essential figure in the management of the P.A.I. is the contact person for the Individual Care Plan, who follows the person from the moment they are taken into care throughout the entire care process. The P.A.I. contact person is an operator is the general practitioner or professional nurse, depending on the prevailing need, identified within the care setting team, who guarantees the entire care process over time, carrying out part of the interventions and interfacing with the various operators and the family, to ensure continuity of care and protection of the non-self-sufficient elderly person. The preparation, verification and updating of the P.A.I. takes place within a team, made up of a generalist doctor and a professional nurse. The patient's general practitioner is automatically part of the team. It is advisable to also involve a family member or carer of the person both for a better understanding of the subject and to compare their expectations with the responses that the Service is able to offer. Responsibility for operational execution lies with the Health Director or, where not foreseen, this operational responsibility is attributed to the coordinator of nursing activities or to the coordinator of protective care activities.

POPULATION/CATCH BASIN

The medical examination service is aimed at all those who do not have the assignment of a general practitioner (tourists) or who wish to have a second opinion; those who cannot or do not want to wait for the appointment with their doctor and more generally to all those who, at any age and for various reasons, wish to have a home visit, possibly.

TYPE AND VOLUME OF SERVICES

The Doctor, with the support of the Nurse, will be able to carry out reading of clinical tests, collection of clinical history, measurement of vital parameters, objective examination, with reference to specific organs and systems (without the use of electro-medical instruments), physiological anamnesis and pathological, request medical prescriptions on a white prescription to be used for a fee at the Health Service and a certificate of illness. For the first period, little demand is expected given that it is an advanced home care service and not yet present in popular habits; once the service has been advertised, at least one call per day is expected for each municipality covered by doctors.
